FMEA, or Failure Mode and Effects Analysis, is a method of error prevention and predictive risk assessment used to identify potential problems and errors in processes, products or services.
The objective of FMEA is to identify where and how a process, product or service can fail and what the possible consequences of these failures are. Subsequently, preventive measures are developed and implemented to minimize the probability of these failures and to minimize their consequences.
Experts from different fields usually perform FMEA in groups to ensure that all aspects of the process, product or service are covered.
FMEA usually consists of five steps:
- Identification of possible errors and their causes
- Assessment of severity and probability of occurrence of errors
- Identification of measures to minimize the probability of errors and their consequences
- Implementation and verification of these measures
- Control and monitoring of the effectiveness of implemented measures.
